Service Agreement

Effective Date

This Service Agreement is effective as of June 07, 2024.

Welcome to Best Fit Counseling & Psychiatry (“we,” “our,” “us”). This Service Agreement (“Agreement”) outlines the terms and conditions governing the use of our mental health services. By accessing or using our services, you agree to be bound by the terms of this Agreement. 

I. Services Provided

We offer a range of mental health services, including but not limited to:

• Individual therapy
• Group therapy
• Telehealth sessions
• Psychological assessments
• Counseling and support services

II. Confidentiality

We are committed to maintaining the confidentiality of your personal health information. All services provided are confidential, and we adhere to all applicable privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA).

III. Payment and Fees

1. Payment Terms: Payment for services is due at the time of the appointment unless otherwise agreed upon.
2. Insurance: We may accept certain insurance plans. It is your responsibility to verify coverage and obtain any necessary authorizations. You are responsible for any co-payments, deductibles, or fees not covered by insurance.
3. Cancellation Policy: Cancellations must be made at least 24 hours in advance. Late cancellations or missed appointments may incur a fee.

IV. Telehealth Services

1. Eligibility: Telehealth services are available to patients residing in areas where we are licensed to provide such services.
2. Technical Requirements: You are responsible for ensuring that you have the necessary equipment and internet connection for telehealth sessions.
3. Consent: By using telehealth services, you consent to the terms outlined in our Telehealth Consent form.

V. Responsibilities of the Patient

As a patient, you agree to:

• Provide accurate and complete information about your health and medical history.
• Participate actively in your treatment plan and follow the recommendations of your healthcare provider.
• Respect the privacy and confidentiality of other patients.
• Keep appointments and notify us in advance if you need to cancel or reschedule.

VI. Limitation of Liability

To the fullest extent permitted by law, Best Fit Counseling & Psychiatry and its affiliates, officers, directors, employees, and agents will not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of our services.

VII. Termination of Services

We reserve the right to terminate services if you fail to comply with the terms of this Agreement or if we determine that it is in your best interest to seek services elsewhere. You have the right to terminate services at any time.

VII. Governing Law and Jurisdiction

This Agreement is governed by and construed in accordance with the laws of the State of Michigan, without regard to its conflict of law principles. You agree to submit to the exclusive jurisdiction of the courts located in Ann Arbor, Michigan, for the resolution of any disputes arising out of or related to this Agreement.

VIII. Changes to This Agreement

We may update this Agreement from time to time. We will notify you of any changes by posting the new Agreement on our website. You are advised to review this Agreement periodically for any changes. Your continued use of our services after any modifications to this Agreement will constitute your acknowledgment of the modifications and your consent to abide by the revised terms.
